GDPR (General Data Protection Regulations)

On the 25th May 2018 a new European Law came into play for all organisations.

 Please notify reception if you are happy for us to contact you via Letter, Telephone Call or Text.

 Failing to do this could affect your health care as Rainworth Surgery is legally required to work within the new regulation and requires explicit consent from our patients for specific forms of contact.

Shingles Vaccine    Please consider a Shingles vaccination if you are 70-73 and your birthday is between 2 September 1942 and 1 September 1946, or 78-79 with your birthday 2 September 1936 - 1 September 1938, ask at Reception for further information.

How to Register as a Patient

  1. In person at the surgery; request Registration Forms from reception for completion.
  2. You can print the registration forms online via the above “Registering Online” page and then bring them in to register.
  3. When registering at the surgery you will need to provide proof of your address by means of a recent utility bill, bank statement, telephone bill and if you have photographic evidence ie Passport, Driving Licence, Bus Pass please bring this with you.

If you need to see a doctor or nurse but haven't yet registered, you can still book an appointment; you just need to come 10 minutes early and register when you get here.

You will not be able to register with the practice if you live outside the practice boundary which is Rainworth and Blidworth.

Preferred Practitioner

You are entitled to see you preferred GP for routine appointments. However, this will not be possible for urgent or emergency appointments that must be seen that day. You may also find that you have to wait longer than usual if you only want to see your preferred GP.

When you register with us you will be assigned to a particular GP, though that doesn’t stop you from expressing a preference to see a different GP when you book appointments. If you would like to be formally assigned to a different GP, you can call the surgery on 01623 794293 and we will be able to make this change for you.
