How to Register as a Patient
- In person – prospective patients can attend Rainworth Surgery to collect registration forms from reception.
- Online – two forms (GMS1 form and New patient registration form) can be printed from our website for prospective patients to complete and then return to Rainworth Surgery.
When registering at the surgery, patients will need to provide proof of address by means of a recent utility bill, bank statement or telephone bill. We also require photographic ID, this can be a passport, driving licence or bus pass. Please being these items with you when dropping your completed forms back to the surgery.
If you do not hold any of the documents listed above, please come and speak to our reception who will be able to provide more help.
You will not be able to register with the practice if you live outside the practice boundary.

If you are unable to complete a form online, please print the below registration form and GMS1 form and bring into the surgery. Please bring with you proof of address with a recent utility bill and if possible photographic ID.
It can take a considerable time for us to receive your medical records and additional information will help us to help you in the meantime.
Please print and complete the Registration Form and New Patient Information Forms below, and bring them to the surgery along with your proof of address in order to register.
